Requirements

• Active and unrestricted RN license in the state of residence.
• Minimum of 2 years of clinical nursing experience.
• Experience with telehealth and remote patient monitoring is preferred.
• Strong communication and interpersonal skills.
• Proficiency with electronic health records (EHR) systems.
• Ability to work independently and manage time effectively.
• Reliable internet connection and a private, quiet workspace.

Responsibilities

• Assess and evaluate patient health conditions, symptoms, and needs through telehealth platforms.
• Provide patient education on disease management, medications, and lifestyle modifications.
• Develop and implement personalized care plans in collaboration with the healthcare team.
• Monitor patient progress and adjust care plans as necessary.
• Maintain accurate and up-to-date patient records in the EHR system.
• Coordinate with other healthcare providers and specialists as needed to ensure comprehensive patient care.

Educational Qualifications

• Associate or Bachelor’s degree in Nursing from an accredited institution.
• Current, active RN license in good standing.
• Advanced certifications (CCRN, CEN, etc.) are a plus.
