Luke's Virtual Hospital exists to optimize acute care outcomes through collaboration and the use of technology. The team-based model of care enables patients in our hospitals to have access to specialty resources, when and where they are needed.

What you can expect from this role

• Remote Patient Management support is available 24 / 7
• Support patients in their homes by virtually monitoring and collaborating in their care
• Patients are given iPad and vital sign equipment for a 30, 60, or 90 day program
• On site position in high tech facility; NOT REMOTE work
• Full-time (0.9 FTE), day shift openings (7a-7p), 12-hour shifts; rotating weekends and holidays
• Certification pay available

Qualifications

• Education : Associate degree required, Bachelor of Nursing Degree preferred
• Experience : 3+ year RN Experience, background in case management helpful
• Licenses / Certifications : Current RN licensure from the State of Practice and Current Basic Life Support (BLS) Provider Certified through American Heart Association or American Red Cross
